Past graduate response
Use this form if you didn’t complete the online Graduate Response when you finished your academic program, and now need to claim your print testamur. (The print testamur option is currently unavailable due to COVID-19. A free digital testamur will be supplied as an alternative.) 

Request a replacement testamur 
(The print testamur option is currently unavailable due to COVID-19. The digital testamur option is still available). Use this form to request a replacement print testamur, or if you completed your academic program between (and including) Semester 1, 1991 to Semester 1, 2017 to request a digital testamur. If you completed your academic program from Semester 2, 2017, you have already been issued with a digital testamur which is hosted in the My Equals portal. 

Change of name and personal details 
Use this form if you are a past graduate and unable to make an online Name Change Request via your Student Centre.

Authentication of USQ graduation documents
(This option is currently unavailable due to COVID-19) Please do not send documents to the University until campuses reopen to students. Use this form if you are submitting original (not copies of) print official graduation documents for authentication by USQ officials. 

Verification letter request
Use this form to request a letter confirming the basic details of your completed academic program.

Ceremony Invitation Letter request
(This option is currently unavailable due to COVID-19). April-May 2020 Ceremonies for Semester 2 and Semester 3 2019 graduands have been cancelled. Use this form if you are a graduand/graduate who has registered and paid the Ceremony Fee to attend a graduation ceremony. This letter can be used to support your own, or your guest’s application for a visa.
